Ellen's I Screen, You Screen Sweeps - Weekdays (12/02/13)

On each episode of the show broadcast during the entry period, an image will appear on-screen. This image will change each Monday through Friday of the entry period.
Following each broadcast, go to the entry form and fill in the following required fields:
(1) Correctly answer the question of the day from the episode of the show that was broadcast on the day you are entering. The question will change each Monday through Friday of the entry period.
(2) Complete the entry form by entering the following information: your first name; last name; email address; home phone number with area code (cell phone number is optional), street address, city, state, zip code, country, date of birth, and verification that you are at least 18 years of age and have read and accept the Official Rules.
(3) Submit an essay of no more than 2000 characters.
(4) Upload a recent photo of yourself.
You will be disqualified if you do not fill in all the required fields each time you enter.
Entry Limit: One (1) entry per person per day per show episode.
Entries submitted on any given day during the entry period must be received between 12:00 a.m. PT on the day of the show’s broadcast until 11:59:59 p.m. PT that same day.
Eligibility: Legal residents who are physically residing in one of the fifty (50) United States or the District of Columbia who are at least eighteen (18) years old as of the date of entry. Residents of Puerto Rico; American Samoa; Guam; all other United States territories, possessions, and protectorates; foreign-based United States embassies and military installations are not eligible to enter. Void wherever restricted or prohibited by law.

Important Additional Information:
The potential prize winners will be notified by phone or email. If the potential prize winners do not accept the prize or return all required documentation and signatures within twenty-four (24) hours following the date and time the prize is awarded, then the potential prize winners will be disqualified and alternate potential prize winners will be selected.
