Weekly ENTRY PERIOD 1 Prize (1): A Bowflex home gym. Approximate Retail Value (“ARV”) of prize: $550.
Weekly ENTRY PERIOD 2 Prize (1): A $250 Dick’s Sporting Goods gift card. ARV of prize: $250.
Weekly ENTRY PERIOD 3 Prize (1): $500 awarded in the form of a check which may be used to hire a personal trainer. ARV of prize: $500.
Weekly ENTRY PERIOD 4 Prize (1): A trip for winner and one (1) guest to Miraval Resort in Tucson, AZ. Trip includes round-trip coach air transportation from major U.S. gateway airport nearest winner’s residence (as determined by sponsor) to destination for winner and one (1) guest, and five (5) nights standard hotel accommodations (one (1) room, double occupancy) at Miraval Resort ARV of prize: $5,000. Actual value may vary depending on city of departure, date of travel/stay. Travel must be completed by 5/1/15 or prize will be forfeited and sponsor will have no further obligation to such winner. If a winner resides within a 100 mile radius of destination, Sponsor reserves the right to provide ground transportation in lieu of air transportation and no compensation or substitution will be provided for difference in prize value. Winner and guest are responsible for obtaining all required travel documents prior to travel. Travel arrangements must be made through Sponsor’s agent, on a carrier of Sponsor’s choice. Travel is subject to availability. Certain blackout dates and travel restrictions may apply. Winner and guest must travel together on the same itinerary.
