Didn't give me a form either. Just said I'd receive my sample in 4-6 weeks! They must be clairvoyant if they know where to send it! LOL!

We are having a problem launching the Infusium Leave-In Treatment 10ml sample giveaway. We are working on getting this up and running as soon as possible. Please stay tuned. I went to get it and its shut down for the weekend, but will start up again on monday
