Here’s how to receive your cash back rebate (up to $7.00 before taxes):
1) PURCHASE one (1) 8 oz. tub Pub Cheese® by Pr?sident® Cheddar & Port Wine flavor.
2) COLLECT the original itemized store-identified cash register receipt(s) dated between 08/01/13 and 10/31/13, and the proof-of-purchase (UPC Symbol). Circle the item description and the purchase price located on the cash receipt. The UPC should be carefully removed from each product once the product package is empty.
3) PRINT AND COMPLETE the official mail-in rebate form, including your name and complete mailing address (city, state, ZIP code), daytime phone number, and a brief description (15 words or less) of why you are dissatisfied with the product.
4) MAIL ALL OF THE ABOVE cash register receipt, proof-of-purchase symbol (UPC Symbol), and official mail-in rebate form, postmarked by 11/14/13, to: Pr?sident® Pub Cheese® Guarantee Offer, PO Box 25723, Rochester, NY 14625.
