Upload a recent picture of yourself that could be shown on-air or online if selected as a contestant.
YOU MUST HAVE ACCESS TO A LAND LINE TELEPHONE IN ORDER TO BE ELIGIBLE.
There will be no more than twenty-one (21) prizes awarded total. One (1) prize will be awarded during each Play Period. Each prize will have an approximate value ranging from $250.00 (two hundred and fifty dollars) to $5,000.00 (five thousand dollars) and will consists of a cash equivalent, such as a check or gift card.
