Refund Information

Offer valid from September 1, 2009 through May 31, 2010, for one (1) BOOST?® Kid Essentials Nutritionally Complete Drink multi-pack, chocolate flavor only.

To redeem, consumer must submit the following items:
(1) Actual UPC from BOOST?® Kid Essentials chocolate flavor outer packaging and
(2) Original product receipt that shows a purchase date between September 1, 2009 and May 31, 2010 of BOOST?® Kid Essentials chocolate with the line item circled or highlighted and
(3) Name and address written on an index card or sheet of paper.
