After successfully completing the form, you will receive an email with the free meter voucher. Get a prescription for either the Accu-Chek Guide or Accu-Chek Guide Me meter and another prescription for Accu-Chek Guide test strips and bring it to a participating retail pharmacy along with the printed voucher (or show your voucher on your smartphone). The pharmacist will then complete processing of the voucher.
