Purchase any food or beverage at a Jimmy John’s restaurant to receive purchase receipt with a survey link. Visit www.telljj.com and enter store number, check number, visit date and time. After that, answer all the survey questions regarding your recent experience at restaurant. After completing the survey, follow the instructions to enter the sweepstakes.

Mail-In (Without Making a Purchase):

Handprint your name (no initials), valid email address, street address (P.O.
