First of all, make a purchase of any specially marked 18-count Frito-Lay Variety Pack from participating store and Scan the QR code on-pack to access entry form or visit www.FLVPBlastOff.com. After that, complete the entry form with your name, complete mailing address (P.O. Boxes are not eligible), phone number, date of birth and email address, and then input your Combined Bag Code printed below the "guaranteed fresh" statement on the front of the bag and the UPC code on the back of the same bag.

Without Purchase (Mail-in):

Write down your personal information including your name, complete address (no P.O. boxes permitted), phone number, date of birth, and email address on a 3" x 5" piece of paper and mail it in a proper postage envelope to affixed to Standard Group, Attn: Frito- Lay Back to School Blast Off Sweepstakes, 500 East Oregon Road, Lititz, PA 17543.
